What happens is that a write to /dev/sg is given a request with non-zero
->iovec_count combined with zero ->dxfer_len. Or with ->dxferp pointing
to an array full of empty iovecs.
Having write permission to /dev/sg shouldn't be equivalent to the
ability to trigger BUG_ON() while holding spinlocks...
Found by Dmitry Vyukov and syzkaller.

During code inspection, while investigating following stack trace
seen on one of the test setup, we found out there was possibility
of memory leak becuase driver was not unwinding the stack properly.
This issue has not been reproduced in a test environment or on a
customer setup.

Both damn things interpret userland pointers embedded into the payload;
worse, they are actually traversing those. Leaving aside the bad
API design, this is very much _not_ safe to call with KERNEL_DS.
Bail out early if that happens.

A race between scanning and fc_remote_port_delete() may result in a
permanent stop if the device gets blocked before scsi_sysfs_add_sdev()
and unblocked after. The reason is that blocking a device sets both the
SDEV_BLOCKED state and the QUEUE_FLAG_STOPPED. However,
scsi_sysfs_add_sdev() unconditionally sets SDEV_RUNNING which causes the
device to be ignored by scsi_target_unblock() and thus never have its
QUEUE_FLAG_STOPPED cleared leading to a device which is apparently
running but has a stopped queue.
We actually have two places where SDEV_RUNNING is set: once in
scsi_add_lun() which respects the blocked flag and once in
scsi_sysfs_add_sdev() which doesn't. Since the second set is entirely
spurious, simply remove it to fix the problem.

30secs before reset

For SRIOV enabled firmware, if there is a OCR(online controller reset)
possibility driver set the convert flag to 1, which is not happening if
there are outstanding commands even after 180 seconds. As driver does
not set convert flag to 1 and still making the OCR to run, VF(Virtual
function) driver is directly writing on to the register instead of
waiting for 30 seconds. Setting convert flag to 1 will cause VF driver
will wait for 30 secs before going for reset.

This patch will fix regression caused by commit 1e793f6fc0db ("scsi:
megaraid_sas: Fix data integrity failure for JBOD (passthrough)
devices").
The problem was that the MEGASAS_IS_LOGICAL macro did not have braces
and as a result the driver ended up exposing a lot of non-existing SCSI
devices (all SCSI commands to channels 1,2,3 were returned as
SUCCESS-DID_OK by driver).

The arcmsr driver failed to pass SYNCHRONIZE CACHE to controller
firmware. Depending on how drive caches are handled internally by
controller firmware this could potentially lead to data integrity
problems.
Ensure that cache flushes are passed to the controller.

Commit 02b01e010afe ("megaraid_sas: return sync cache call with
success") modified the driver to successfully complete SYNCHRONIZE_CACHE
commands without passing them to the controller. Disk drive caches are
only explicitly managed by controller firmware when operating in RAID
mode. So this commit effectively disabled writeback cache flushing for
any drives used in JBOD mode, leading to data integrity failures.

This patch fixes one use-after-free report[1] by KASAN.
In __scsi_scan_target(), when a type 31 device is probed,
SCSI_SCAN_TARGET_PRESENT is returned and the target will be scanned
again.
Inside the following scsi_report_lun_scan(), one new scsi_device
instance is allocated, and scsi_probe_and_add_lun() is called again to
probe the target and still see type 31 device, finally
__scsi_remove_device() is called to remove & free the device at the end
of scsi_probe_and_add_lun(), so cause use-after-free in
scsi_report_lun_scan().
And the following SCSI log can be observed:

In aacraid's ioctl_send_fib() we do two fetches from userspace, one the
get the fib header's size and one for the fib itself. Later we use the
size field from the second fetch to further process the fib. If for some
reason the size from the second fetch is different than from the first
fix, we may encounter an out-of- bounds access in aac_fib_send(). We
also check the sender size to insure it is not out of bounds. This was
reported in https://bugzilla.kernel.org/show_bug.cgi?id=116751 and was
assigned CVE-2016-6480.

sas_ata_strategy_handler() adds the works of the ata error handler to
system_unbound_wq. This workqueue asynchronously runs work items, so the
ata error handler will be performed concurrently on different CPUs. In
this case, ->host_failed will be decreased simultaneously in
scsi_eh_finish_cmd() on different CPUs, and become abnormal.
It will lead to permanently inequality between ->host_failed and
->host_busy, and scsi error handler thread won't start running. IO
errors after that won't be handled.
Since all scmds must have been handled in the strategy handler, just
remove the decrement in scsi_eh_finish_cmd() and zero ->host_busy after
the strategy handler to fix this race.

When SCSI was written, all commands coming from the filesystem
(REQ_TYPE_FS commands) had data. This meant that our signal for needing
to complete the command was the number of bytes completed being equal to
the number of bytes in the request. Unfortunately, with the advent of
flush barriers, we can now get zero length REQ_TYPE_FS commands, which
confuse this logic because they satisfy the condition every time. This
means they never get retried even for retryable conditions, like UNIT
ATTENTION because we complete them early assuming they're done. Fix
this by special casing the early completion condition to recognise zero
length commands with errors and let them drop through to the retry code.

When KDUMP is triggered the driver first talks to the firmware in INTX
mode, but the adapter firmware is still in MSIX mode. Therefore the first
driver command hangs since the driver is waiting for an INTX response and
firmware gives a MSIX response. If when the OS is installed on a RAID
drive created by the adapter KDUMP will hang since the driver does not
receive a response in sync mode.
Fixed by: Change the firmware to INTX mode if it is in MSIX mode before
sending the first sync command.

Typically under error conditions, it is possible for aac_command_thread()
to miss the wakeup from kthread_stop() and go back to sleep, causing it
to hang aac_shutdown.
In the observed scenario, the adapter is not functioning correctly and so
aac_fib_send() never completes (or time-outs depending on how it was
called). Shortly after aac_command_thread() starts it performs
aac_fib_send(SendHostTime) which hangs. When aac_probe_one
/aac_get_adapter_info send time outs, kthread_stop is called which breaks
the command thread out of it's hang.
The code will still go back to sleep in schedule_timeout() without
checking kthread_should_stop() so it causes aac_probe_one to hang until
the schedule_timeout() which is 30 minutes.
Fixed by: Adding another kthread_should_stop() before schedule_timeout()

aac_fib_send has a special function case for initial commands during
driver initialization using wait < 0(pseudo sync mode). In this case,
the command does not sleep but rather spins checking for timeout.This
loop is calls cpu_relax() in an attempt to allow other processes/threads
to use the CPU, but this function does not relinquish the CPU and so the
command will hog the processor. This was observed in a KDUMP
"crashkernel" and that prevented the "command thread" (which is
responsible for completing the command from being timed out) from
starting because it could not get the CPU.
Fixed by replacing "cpu_relax()" call with "schedule()"

During revalidate we check whether device capacity has changed before we
decide whether to output disk information or not.
The check for old capacity failed to take into account that we scaled
sdkp->capacity based on the reported logical block size. And therefore
the capacity test would always fail for devices with sectors bigger than
512 bytes and we would print several copies of the same discovery
information.
Avoid scaling sdkp->capacity and instead adjust the value on the fly
when setting the block device capacity and generating fake C/H/S
geometry.

One of the strange things that the original sg driver did was let the
user provide both a data-out buffer (it followed the sg_header+cdb)
_and_ specify a reply length greater than zero. What happened was that
the user data-out buffer was copied into some kernel buffers and then
the mid level was told a read type operation would take place with the
data from the device overwriting the same kernel buffers. The user would
then read those kernel buffers back into the user space.
From what I can tell, the above action was broken by commit fad7f01e61bf
("sg: set dxferp to NULL for READ with the older SG interface") in 2008
and syzkaller found that out recently.
Make sure that a user space pointer is passed through when data follows
the sg_header structure and command. Fix the abnormal case when a
non-zero reply_len is also given.

aac_fib_map_free() calls pci_free_consistent() without checking that
dev->hw_fib_va is not NULL and dev->max_fib_size is not zero.If they are
indeed NULL/0, this will result in a hang as pci_free_consistent() will
attempt to invalidate cache for the entire 64-bit address space
(which would take a very long time).
Fixed by adding a check to make sure that dev->hw_fib_va and
dev->max_fib_size are not NULL and 0 respectively.

The driver utilizes an array of atomic variables to keep track of IO
submissions to each vector. To submit an IO multiple threads iterate
through the array to find a vector which has empty slots to send an
IO. The reading and updating of the variable is not atomic, causing race
conditions when a thread uses a full vector to submit an IO.
Fixed by mapping each FIB to a vector, the submission path then uses
said vector to submit IO thereby removing the possibly of a race
condition.The vector assignment is started from 1 since vector 0 is
reserved for the use of AIF management FIBS.If the number of MSIx
vectors is 1 (MSI or INTx mode) then all the fibs are allocated to
vector 0.

As result of deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range() or pci_enable_msi_exact()
and pci_enable_msix_range() or pci_enable_msix_exact()
interfaces.
Log message code 0x00c6 preserved, although it is reported
after successful call to pci_enable_msix_range(), not before
possibly unsuccessful call to pci_enable_msix(). Consumers
of the error code should not notice the difference.

Without in-depth understanding of SCSI layer and the Marvell controller,
I suspect this happens because when the link goes down (because of an
error) we schedule scsi_rescan_device() which again fails to read VPD
data... ad infinitum.
Since VPD data cannot be read from the device anyway we prevent the SCSI
layer from even trying by blacklisting the device. This gets away the
error and the system starts up normally.

If MODE SELECT returns with sense '05/91/36' (command lock violation)
it should always be retried without counting the number of retries.
During an HBA upgrade or similar circumstances one might see a flood
of MODE SELECT command from various HBAs, which will easily trigger
the sense code and exceed the retry count.

The newly introduced aborted_task TFO callback has to terminate
exchange with QLogic driver, since command is being deleted and
no status will be queued to the driver at a later point.
This patch also moves the burden of releasing one cmd refcount to
the aborted_task handler.
Changed iSCSI aborted_task logic to satisfy the above requirement.

RSCN processing in qla2xxx driver can run in parallel with ELS/IO
processing. As such the decision to remove disappeared fc port's
session could be stale, because a new login sequence has occurred
since and created a brand new session.
Previous mechanism of dealing with this by delaying deletion request
was prone to erroneous deletions if the event that was supposed to
cancel the deletion never arrived or has been delayed in processing.
New mechanism relies on a time-like generation counter to serialize
RSCN updates relative to ELS/IO updates.

If a new initiator (different WWN) shows up on the same fcport, old
initiator's session is scheduled for deletion. But there is a small
window between it being marked with QLA_SESS_DELETION_IN_PROGRESS
and qlt_unret_sess getting called when new session's commands will
keep finding old session in the fcport map.
This patch drops cmds/tmrs if they find session in the progress of
being deleted.

- keep qla_tgt_sess object on the session list until it's freed
- modify use of sess->deleted flag to differentiate delayed
session deletion that can be cancelled from irreversible one:
QLA_SESS_DELETION_PENDING vs QLA_SESS_DELETION_IN_PROGRESS
- during IN_PROGRESS deletion all newly arrived commands and TMRs will
be rejected, existing commands and TMRs will be terminated when
given by the core to the fabric or simply dropped if session logout
has already happened (logout terminates all existing exchanges)
- new PLOGI will initiate deletion of the following sessions
(unless deletion is already IN_PROGRESS):
- with the same port_name (with logout)
- different port_name, different loop_id but the same port_id
(with logout)
- different port_name, different port_id, but the same loop_id
(without logout)
- additionally each new PLOGI will store imm notify iocb in the
same port_name session being deleted. When deletion process
completes this iocb will be acked. Only the most recent PLOGI
iocb is stored. The older ones will be terminated when replaced.
- new PRLI will initiate deletion of the following sessions
(unless deletion is already IN_PROGRESS):

Since cmds go into qla_tgt_wq and TMRs don't, it's possible that TMR
like TASK_ABORT can be queued over the cmd for which it was meant.
To avoid this race, use a per-port list to keep track of cmds that
are enqueued to qla_tgt_wq but not yet processed. When a TMR arrives,
iterate through this list and remove any cmds that match the TMR.
This patch supports TASK_ABORT and LUN_RESET.

We shouldn't try to migrate pages in sg VMA as we don't have a way to
update Sg_scatter_hold::pages accordingly from mm core.
Let's mark the VMA as VM_IO to indicate to mm core that the VMA is not
migratable.

Runtime suspend during driver probe and removal can cause problems.
The driver's runtime_suspend or runtime_resume callbacks may invoked
before the driver has finished binding to the device or after the
driver has unbound from the device.
This problem shows up with the sd and sr drivers, and can cause disk
or CD/DVD drives to become unusable as a result. The fix is simple.
The drivers store a pointer to the scsi_disk or scsi_cd structure as
their private device data when probing is finished, so we simply have
to be sure to clear the private data during removal and test it during
runtime suspend/resume.

Another iscsi target that cannot handle large IOs, but does not tell us
a limit.
The Synology iSCSI targets report:

and the size of the command it can handle seems to depend on how much
memory it can allocate at the time. This results in IO errors when
handling large IOs. This patch just has us use the old 1024 default
sectors for this target by adding it to the scsi blacklist. We do not
have good contacs with this vendors, so I have not been able to try and
fix on their side.
I have posted this a long while back, but it was not merged. This
version just fixes it up for merge/patch failures in the original
version.

When pci_pool_alloc fails in mvs_task_prep then task->lldd_task stays
NULL but it's later used in mvs_abort_task as slot which is passed
to mvs_slot_task_free causing NULL pointer dereference.
Just return from mvs_slot_task_free when passed with NULL slot.

bounce buffer them. Add a helper to identify these commands and don't

b9d5c6b7ef57 ("[SCSI] cleanup setting task state in
scsi_error_handler()") has introduced a race between scsi_error_handler
and scsi_host_dev_release resulting in the hang when the device goes
away because scsi_error_handler might miss a wake up:

fnic_queuecommand() to avoid deadloack

We added changes in fnic driver patch 1.6.0.16 to acquire
io_req_lock in fnic_queuecommand() before issuing I/O so that io completion
is serialized. But when releasing the lock we check for the I/O flag and
this could be modified if IO abort occurs before I/O completion. In this case
we wont release the lock and causes deadlock in some scenerios. Using the
local variable to check the IO lock status will resolve the problem.

The routines in scsi_rpm.c assume that if a runtime-PM callback is
invoked for a SCSI device, it can only mean that the device's driver
has asked the block layer to handle the runtime power management (by
calling blk_pm_runtime_init(), which among other things sets q->dev).
However, this assumption turns out to be wrong for things like the ses
driver. Normally ses devices are not allowed to do runtime PM, but
userspace can override this setting. If this happens, the kernel gets
a NULL pointer dereference when blk_post_runtime_resume() tries to use
the uninitialized q->dev pointer.
This patch fixes the problem by calling the block layer's runtime-PM
routines only if the device's driver really does have a runtime-PM
callback routine. Since ses doesn't define any such callbacks, the
crash won't occur.
This fixes Bugzilla #101371.

Commit bcdb247c6b6a ("sd: Limit transfer length") clamped the maximum
size of an I/O request to the MAXIMUM TRANSFER LENGTH field in the BLOCK
LIMITS VPD. This had the unfortunate effect of also limiting the maximum
size of non-filesystem requests sent to the device through sg/bsg.
Avoid using blk_queue_max_hw_sectors() and set the max_sectors queue
limit directly.
Also update the comment in blk_limits_max_hw_sectors() to clarify that
max_hw_sectors defines the limit for the I/O controller only.
